Chroma Case Management (CCM) is a case management company of highly qualified health professionals who specialise in coordinating personalised rehabilitation programmes for clients who have sustained severe injuries, most often as a result of incidents in the workplace or road traffic accidents.

Who We Are

Chroma Case Management – making a difference

CCM’s case managers are highly experienced and caring registered healthcare professionals who conduct holistic assessments of clients’ needs, coordinate and implement personalised rehabilitation pathways and work together with them to achieve their goals for recovery.

Operating throughout England and Wales, clients are most often referred to us through claimant solicitors and/or insurance companies as part of an ongoing compensation claim, but self-referrals are also welcome.

At CCM our quality assurance relies on the depth of care we apply to our clients, customers, suppliers and business. We take feedback very seriously and aspire to develop and improve on the recommendations of those who engage with us to improve our service.

We have a robust clinical governance framework and all our case managers are all accredited by either the Health & Care Professions Council (HCPC) or the Nursing & Midwifery Council (NMC). In addition, we also require our case managers to be registered with the Case Management Society of the UK (CMSUK) and/or the British Association of Brain Injury Case Managers (BABICM).

Psychosocial Rehabilitation

CCM operates a biopsychosocial approach to rehabilitation. As well as addressing clients’ medical needs, CCM:

• Recognises the importance of psychological health and embeds psychological treatment principles within clients’ rehabilitation programmes. Clients are also able to access cognitive and other evidence-based therapies.

• Incorporates clients’ personal interests into their rehabilitation programmes to enhance their motivation, engagement, wellbeing and overall outcomes.

• Aims to facilitate clients’ wellbeing and recovery by addressing issues such as sleep disturbance, poor nutrition and lack of exercise.

What We Do

CCM and how we make a difference

Immediate Needs Assessments (INA)

The INA forms the basis of a client’s rehabilitation programme. Conducted by an experienced case manager, this specialist assessment allows us to identify the client’s rehabilitation requirements and prepare a comprehensive INA report. This INA report includes a personalised Rehabilitation and Recovery Plan (RRP), designed specifically for that client with the aim of returning them to as close as possible to their pre-injury health status and lifestyle. This could include returning to work, study, leisure pursuits and activities of daily living (ADLs). The INA also facilitates the development of a strong and trusting relationship between the client and their case manager, which is a vital part of the rehabilitation process.

The assessment usually takes about 2 hours and can be carried out in the client’s home or when appropriate, over the telephone.

•  2 hour (approx.) holistic assessment
•  Client-centred approach
•  Comprehensive report incl. bespoke Rehabilitation and Recovery Plan (RRP)
•  Ongoing case management if required and approved

The RRP is a comprehensive plan which outlines in clear and simple language the client’s rehabilitation goals, as well as the recommended actions, estimated costs and likely timescales required to help the client to achieve these goals.

Case Management

If CCM are given funding authorisation to proceed, the case manager will work collaboratively with the client to implement their unique Rehabilitation and Recovery Plan (RRP) in order to achieve their personal goals. This may involve:

•  Multidisciplinary liaison with NHS, social care and private providers
•  Collaboration with employers to facilitate the client’s return to work
•  Working closely with the client to facilitate participation in meaningful activity
•  Devising creative solutions to address any unmet needs

CCM case managers maintain close and regular contact with the client and their families to monitor and review progress against agreed goals.

Rehabilitation and Recovery Plans are reviewed and updated on a regular basis.

Vocational Rehabilitation

CCM believes that enabling clients to return to work, purposeful activity or work readiness at an early stage in their rehabilitation is important for their health and recovery.

One of the primary aims of most rehabilitation programmes is therefore to facilitate clients’ vocational goals. This relies not only on their physical capacity, but also on their psychological wellbeing.

CCM case managers work closely with clients, employers and, if appropriate, specialist functional and vocational consultants to implement return to work options that inspire and motivate their clients.

Wendy Thomas
Managing Director
Wendy is a qualified dramatherapist (2015) and is a Director of CCM. In addition to her dramatherapy training, Wendy has a commercial background working in financial services in sales and key account business development. Wendy is also the Joint Managing Director of CHROMA.

Victoria is a qualified occupational therapist who has specialised in case management since 2008. She has worked as a case manager and operations manager at a number of leading case management companies within the UK, across the injury acuity spectrum. She also sits on the CMSUK Board of Directors.

Finance Director
Andrew is a qualified accountant. His financial experience has been gained in the commercial sector where was FD for the UK’s largest independent floor-coverings buying group. He has also worked in schools and provided business development consultancy in the SME sector.


Liz Turley
Operations Manager                                                                                              Liz has a diverse background in administration having worked in the service industry and education prior to joining Chroma.

Liz oversees the administration at CCM.


Jenny Jenkins
Administration Support

Jenny is an experienced administrator who, prior to joining Chroma, worked for over 20 years in a major high street bank.

Jenny works part time at CCM in an administrative role.
